Interactive patient forms

When we install interactive services on a website we use SSL technology, this means that any information transmitted from your web site will be sent as an encrypted message making it virtually impossible to be read by 3rd parties.
To enable these services on your web site you will need to provide us with an email address where the forms will be sent. It is recommended you use your NHS email as this uses a secure medium to store email, IT governance & best practice dictates that care of patient information be a priority.
1) Secure online registration
If you would like to to offer an online patient registration service we will set up a secure, automated & encrypted email system that works directly from your website. We will work with you to define the fields you need the patient to complete, these can be required (answer) fields, free flow text forms, multiple choice drop down selection. When the user submits the form from your website you will receive an email notifying you of the new registration. The embedded security feature ensures no one but you will be able to read the email.
2) Secure change of address / change of name forms
We can deploy a secure change of details page where patients can notify you of their updated personal information. This is sent to you via encrypted secure email, it cannot be read by 3rd parties.
3) Secure repeat prescription form
Provide your patients with the opportunity to submit to you their repeat prescription via the website. Any information included on this form is encrypted so cannot be read by anyone other than you.
